Please note that communication at our clinic may move a bit more slowly than you may be accustomed to in a conventional medical setting. We intentionally maintain a quieter, more personalized pace of care in order to preserve the depth and quality of each patient interaction.
Tania, our scheduling assistant, typically responds to emails and phone calls twice daily and is not available during all business hours. Email is generally the fastest way to reach us, while phone messages may take a bit longer to return. You can typically expect a response within 24 hours during business days.
For urgent matters, Dr. Caselli maintains an urgent pager line at 530-918-8486. Please note that use of the pager for medical questions or clinical concerns may incur a fee. Reasonable logistical or time-sensitive non-medical issues — such as difficulty locating the office or running late to an appointment — are not subject to this charge.

Unlike traditional insurance-based practices that bill based on procedures and diagnostic codes, our practice utilizes a time-based care model. Your fee reflects the physician's total professional time devoted to your care, including preparation, consultation, clinical decision-making, documentation, review of records and results, care coordination, and follow-up management.
This model allows for thoughtful, individualized care without the limitations imposed by insurance-driven visit structures.
Your Visit Fee Includes
Review of intake forms, questionnaires, and medical history
Review of prior records, specialist notes, laboratory results, and imaging reports you provide
Clinical preparation and treatment planning before your appointment
Time spent during your consultation, including assessment, education, counseling, and shared decision-making
Development and adjustment of personalized treatment plans
Communication of treatment plan recommendations and follow-up instructions after your visit
Ordering and interpretation of laboratory testing and imaging studies discussed in your visit
*lab or imaging orders requested outside of a visit have an additional charge
Prescription management, including new prescriptions discussed in your visit and routine refills
Documentation, charting, and maintenance of your medical record
Review of test results received after your visit
Coordination of care with pharmacies, laboratories, imaging centers, and other healthcare providers when clinically necessary
Referrals to specialists or other healthcare services
Prior Authorization coordination when required
Our fees reflect the total professional time devoted to your care before, during, and after your appointment—not solely the time spent face-to-face during the visit.

Brief visit: 15–30 minutes
Ideal for straightforward follow-ups, medication management, hormone optimization or weight-loss check-ins, review of a limited number of laboratory results, or discussion of a single concern.
Standard visit: 45–60 minutes
Appropriate for addressing multiple concerns, more comprehensive laboratory review, treatment plan adjustments, symptom evaluation, and more in-depth discussion of health goals and progress.
Extended visit: 75–90 minutes
Best suited for new patients, patients returning after an extended absence, complex medical histories, comprehensive laboratory review, or situations requiring extensive evaluation, counseling, and treatment planning.
When scheduling, consider the number and complexity of topics you'd like to address.
Because it can be difficult to predict exactly how much time will be needed, appointments are billed based on the actual time used—not the time reserved. If your visit takes less time than scheduled, you will only be charged for the time utilized.
We strive to stay on schedule while ensuring your concerns are fully addressed. When scheduling permits, we will do our best to continue important discussions if additional time is needed rather than ending the visit abruptly.

Eligibility Guidelines for Income-Based Adjusted Rate (approximate):
1-person household: up to ~$85,000
2-person household: up to ~$115,000

The cost is determined by your income and household size. Most people will qualify for the Income-Based Adjusted Rate. Patients above these ranges are billed at the standard rate listed below.

This helps us assign the appropriate pricing tier. You do not need to provide any proof of income. You determine your category based on income and household size and let us know where you fit. We have a worksheet that will guide you during your intake process.

Payment is collected at the time of service. We do not accept or bill any insurance.
We accept HSA and FSA funds, along with standard credit and debit payments.
Many patients with out-of-network benefits are able to receive partial reimbursement depending on their plan. In most cases, insurance will also cover most standard laboratory testing, imaging, prescriptions, and referrals ordered.

Upon request, a detailed superbill can be provided at the end of each visit. This may be submitted to insurance for potential reimbursement, application toward deductibles, or out-of-pocket maximums depending on individual coverage.
